Liability Insurance
All outside users require a minimum of $2 million liability insurance.
Provide Your Own Liability Insurance
All outside users require a minimum of $2 million liability insurance.
Before uploading, please ensure your Certificate of Insurance:

Lists the same organization or group name provided on this application as the Named Insured

Provides liability coverage of at least $2,000,000

Is valid for all requested dates of facility use

Lists Park West School Division as an additional insured with respect to your use of the facility
Certificates that do not meet the above requirements will need to be corrected before facility use can be approved.
Purchase Liability Insurance
If you require liability insurance, please select this option and submit your request. Once your facility request has been reviewed and approved, you will receive an email with instructions to purchase the required coverage and complete payment.
